(e)     A person if his or her parent or someone else with lawful

1.      Application for Student Admission (will be provided by this office);
3 T9 t/ p" l# M/ e1 B2.      Copies of passports of parent(s) and child(ren);
: V; }% G+ T1 C/ C3.      Birth certificate of child(ren) / Family Register (translated
; p, N# Q' o& {into English);
4.      Proof of residential address / tenancy (eg lease agreement/tax
bill/utility bill - driver’s license not acceptable);
5.      Documentation to verify child(ren)’s first ever date of entry
! R. Q( B2 A' _& [4 m; Pinto Canada (eg. passport stamp / immigration documents / airline

6.      Copies of all immigration documents received ie. Work Permit,

require.
