(e)     A person if his or her parent or someone else with lawful
custody of him or her is in Canada:

application for a work permit under the Immigration and Refugee
Protection Act (Canada),

- u* \7 z4 R' r' @  j# V. D( n$ e/ l1.      Application for Student Admission (will be provided by this office);
2.      Copies of passports of parent(s) and child(ren);
1 X) b' b; G3 W5 i3.      Birth certificate of child(ren) / Family Register (translated
into English);
* U: }! W  K# B! A& |; p/ z. z4.      Proof of residential address / tenancy (eg lease agreement/tax
bill/utility bill - driver’s license not acceptable);
5.      Documentation to verify child(ren)’s first ever date of entry
1 ?; J4 X/ w3 {% x* ointo Canada (eg. passport stamp / immigration documents / airline
( A; W7 t5 [. H) [) V) \8 E2 {ticket etc.);
% C/ |! L% j- a6.      Copies of all immigration documents received ie. Work Permit,
$ m# {9 }! N8 U) `Study Permit,  Visitor Records and any other documents we might
require.
